经寰椎后弓侧块螺钉固定治疗上颈椎损伤

目的 评估经寰椎后弓侧块螺钉固定应用于上颈椎损伤的价值和作用。方法 对13例上颈椎损伤患者进行后路经寰椎后弓侧块行钉棒系统复位内固定,植骨融合,重建上颈椎的稳定性。结果 13例获2~18个月随访,无术中、术后并发症,头部前曲、后伸活动好,左右旋转活动有受限,未见螺钉松动及钉棒断裂现象。结论经寰椎后弓侧块螺钉固定可使固定节段获得高度的内在稳定性,同时保留枕颈间的运动功能,并发症少,适用于上颈椎损伤需重建稳定性的治疗。

Internal Fixation Via the Posterior Bow Side Mass of Atlas for the Treatment of the Upper Cervical Spine Injuries

Objective To evaluate the effectiveness of internal fixation via the posterior bow side mass of atlas for the treatment of the upper cervical spine injuries. Methods Reduction and internal fixation via the posterior bow side mass of atlas with bone graft was performed on 1 case of the fracture of posterior bow of atlas together with the fracture of axis arch, 5 cases of the fracture of axis arch, 4 cases of the fracture of middle odontoid process of axis and 2 cases of the fracture in the base of odontoid process of axis. Results Thirteen cases were followed up for 2 to 13 monthes. No operative and postoperative complications occurred such as loosening and breaking of the instrumentations. The followed - up results showed no limitation of flexion - extension movememnt of the neck, but with restriction in rotation of the neck. Conclusion Internal fixation via the posterior bow side mass of atlas can obtain good stability of the fixed segmem-nts without affecting the occipo - cervical movememnt. It is an ideal method for the reconstruction of the stability of the upper cervical spine.
