锁骨钩钢板固定与克氏针张力带固定治疗肩锁关节脱位疗效比较

目的比较锁骨钩钢板与克氏针张力带手术治疗新鲜肩锁关节脱位(TossyⅢ)的疗效。方法40例新鲜肩锁关节脱位,应用锁骨钩钢板10例,克氏针张力带30例,术后随访1年以上进行比较。结果随访1~5年(平均2.0年),按Karlsson评定标准,10例钩钢板病例中优5例,良4例,差1例,优良率90%;30例克氏针张力带病例中优12例,良9例,差9例,优良率70%;两组优良率有明显差异(P<0.01)。克氏针张力带组并发病较多,钩钢板组无并发症。结论锁骨钩钢板治疗重度肩锁关节脱位具有疗效显著,并发症少等优点,值得临床推广。

Comparison of efficacy of clavicular hook plate internal fixation and tension band wiring for treatment of acromioclavicular joint dislocation

Objective To compare efficacy of clavicular hook plate and tension band wiring for treaatment of acromioclavicular joint dislocation(TossyⅢ).Methods Forty patients with fresh acromioclavicular joint dislocation were treated with clavicular hook plate(10 cases) and tension band wiring(30 cases).Follow up of over 1 year was performed to compare the efficacy between the two methods after the operation.Results A follow up of 1 to 5 year(mean 2.0 years),according to Karlsson evaluation criterion,among of 10 cases performed with clavicular hook plate,5 cases were excellent,4 cases were good and 1 case was poor,excellent and good rate was 90%.Among of 30 cases treated with tension band wiring,12 cases were excellent,9 cases were good and 9 cases were poor,excellent and good rate was 70%.There was predominant difference in excellent and good rate in two groups(P<0.01).There were more complications in the group performed with tension band wiring and none complication occurred in the group performed with calvicular hook plate.Conclusion There are many advantages of clavicular hook plate for the treatment of severe acromioclavicular joint dislocation,such as remarkable efficacy and few complications.It is worthy of clinical application.
