高血压病患者的行为心理与门诊治疗的依从性探讨

目的：探讨高血压病患的行为心理与门诊治疗的依从性关系。方法：通过前瞻性量表调查方式，对患进行为期6月～10年，间隔1～8周的门诊治疗随访调查，判断患行为心理与执行医嘱依从性的关系及对血压控制程度的影响。结果：616例患，560例(90．9％)完成随访调查，56例(9．1％)失访。完成随访调查中，137例(24．5％)执行医嘱依从性好，其血压均得到满意控制；423例(75．5％)执行医嘱依从性差．主要原因是对高血压知识缺乏了解，主观不重视，医嘱太复杂，其血压控制满意仅93例(22．0％)，141例(33．3％)因血压反复波动或出现心脑血管并发症而住院或再次住院治疗。结论：高血压病患对高血压的认识、对治疗的重视及医嘱的复杂性与门诊治疗的依从性直接相关，决定了疾病治疗的成败。

Correlation between behavior-psychological factor and compliance of the outpatient treatment in essential hypertension

Objective: To study the correlation between behavior-psychological factor and compliance of the outpatient treatment in essential hypertension. Methods: To evaluate the correlation among behavior-psychological factor and outpatient treatment compliance and blood pressure degree that was controled. Survey was penformed by prospecrive measuring scale for a period of 6-10 years and separate from 1 - 8 weeks. Results: In 616 cases, 560 (90. 9%) cases completed and 56 (9.1%) cases were not completed survey. For patients completed survey the compliance of outpatient treatment was fine in 137 (24. 5%) cases, and the blood pressure was controlled satisfactorily. But in 423 cases (75. 5%), the compliance of outpatient treatment was bad, and the blood pressure was controlled satisfactorily only in 93 cases (22. 0%). Its main cause was that, knowledge of hypertension was very few (81.3%), physician order was very complicated (79. 4%), treatment was despised (49. 4%) Conclusion: There is correlation among compliance of outpatient treatment and knowledge of hypertension, rationality of physician order, pay attention to treatment.
