糖尿病患者视网膜中央动脉血流动力学与糖化血红蛋白的关系

摘    要:目的:了解糖尿病患者视网膜中央动脉(CRA)血流动力学变化与糖化血红蛋白(HbA1c)的关系,探讨糖尿病视网膜血流变化的规律及影响因素。方法:用彩色多普勒超声检测66例糖尿病患者和30例正常对照者的CRA收缩期最高血流速度(Vmax),舒张末期最低血流速度(Vmin),阻力指数(RI),搏动指数(PI),用免疫凝集抑制法测定全部患者的HbA1c.结果:糖尿病患者CRA的Vmax,Vmin均低于正常组(P<0.05),RI高于正常组(P<0.05)。HbA1c与糖尿病患者CRA的血流速度呈负相关,均低于呈正相关。结论:糖尿病患者存在视网膜中央动脉血流动力学异常,且与血糖水平有关,控制血糖有益于改善视网膜缺血及缺氧状态。

Abstract:Objective To understand the relationship between blood flow changes of central retinal artery (CRA) and HbA 1c levels in the patients with diabetes mellitus (DM).To study the rules of blood flow velocity changes in diabetes mellitus and the factors of influence. Methods Blood flow changes of central retinal artery in 66 patients with dibetes mellitus and 30 normal volunteers were measured with color Doppler imaging technique. The peak stystolic velocities(Vmax), the end diastolic velocities(Vmin), the resistance indexes(RI) and the pulsatility indexes(PI) were compared. HbA 1c levels were tested with immunosuppressive method in all patients. Results Peak systolic and end diastolic velocities of CRA in DM patients were lower than that of the normal(P< 0.05 ), but the RI was increased(P<0.05). In the DM patients,the blood flow velocity of CRA was correlated with HbA 1c negatively, but the RI was correlated with HbA 1c positively. Conclusion Some blood flow changes of CRA in patients with diabetes mellitus are present,they are related to glycemic levels. The control of hyper-glycemic levels can improve ischemia and hypoxia in diabetic refinopathy.
