肝移植病人术后感染的病原菌分布及其耐药性分析

目的 探讨肝移植病人术后感染的病原菌分布及其耐药性。方法对我院2000．1—2002．12期间肝移植病人术后送检的各类标本中分离出的1002株病原菌的分布及耐药性进行回顾性分析。结果 肝移植病人术后感染的病原菌以革兰阴性杆菌为主占55．9％，其次为革兰阳性球菌(26．7％)和真菌(17．4％）。最常见的5个菌种依次是表皮葡萄球菌、嗜麦芽窄食单胞菌、鲍曼不动杆菌、铜绿假单胞菌、肺炎克雷伯菌、病原菌分离部位以痰、血液、胆汁为主，占85．2％。细菌的耐药现象严重，常见菌种基本上都呈多重耐药，但革兰阴性杆菌对泰能耐药率相对较低。没有发现耐万古霉素的葡萄球菌。结论 加强肝移植病人术后的细菌分离及耐药性监测，对于合理使用抗生素预防和治疗感染十分重要。

Distribution and drug resistance of infection pathogens in patients of post-opof liver transplantation

Objective To explore the distribution and drug resistance of infection pathogens in of patients post-op of liver transplantation.Methods T he distribution and drug resistance of1002pathogens isolated from various sp ecimen in liver transplantation patients were retrospectively analyzed during2 000.1-2002.12.Results Pathogens of in-fection in liver transplant ation unit is dominated by the Gram-negative bacilli (55.9%),secondly t he Gram-positive coccus(26.7%)and fungus(17.4%).The most frequen tly seen strains in successive are Staphylococcus epidermidis,Stenotrophomonas maltophilia,Acinetobacter baumannii,Pseudomonas aeruginosa,Klebsiella pneumon iae.The sites of pathogens isolation were dominated by sputa,blood,bile(85 .2%).The drug resistance of bacteria was very severe with the usually seen stains being basically multi-drug resis-tance,But the rate of drug resistanc e of Gram-negative bacilli to Imipenem was relative-ly low.No strain of Stap hylococcus resisting to vancomycin was found.Conclusions Strengthening bacteri a isolation and drug resistances monitor in patients of post-op of liver transp lantation are very important to appropriate use of antibiotics.
