液基细胞学与巴氏细胞学筛查宫颈癌的对比研究

目的　评价ThinPrep宫颈液基细胞学检查在宫颈癌筛查中的敏感性和价值。方法　对652例有临床 症状的患者做宫颈脱落细胞巴氏涂片和液基细胞薄片,细胞学诊断采用TBS标准,以组织学活检作对照。结果 　液基细胞学和巴氏细胞学分别检出鳞状细胞癌(SCC)100%、80%,鳞状上皮内高度病变(HSIL)91.0%、 71.8%,鳞状上皮内低度病变(LSIL)72.1%、51.5%,两者诊断HSIL或以上的准确率分别为91.6%和72.3%。 结论　ThinPrep液基细胞学检查敏感性高于巴氏细胞学,能提高检出率,特别是宫颈癌早期和HSIL。

Comparative study on the liquid-based cytology and Pap smear cytology in the screening of cervical cancer

Objective To evaluate the sensitivity and clinical value of liquid-based cytology(ThinPrep cytology test) in the screening of cervical cancer. Methods 652 cases with cervical erosion, irregular vaginal bleeding and/or cervical condyloma were tested by Pap smear and ThinPrep cytology and diagnosed with TBS standard. The positive results were compared with those of colposcopic biopsics. Results The detection rates by ThinPrep cytology and Pap smear were 100%,80% for SCC, 91.0%,71.8% for HSIL and 72.1%, 51.5% for LSIL. The detective rates in HSIL and above were 91.6% and 72.3%. Conclusions ThinPrep cytology test is more sensitive than Pap smear. Liquid-based cytology can improve the detection rate of LSIL in cervical screen, especially in early cervical carcinoma and HSIL.
