B7-H4、P53、CEA在胃癌中的表达及临床意义

目的探讨B7-H4、P53、CEA在胃癌中的表达特点及临床意义,寻找胃癌早期诊断的免疫学指标。方法用免疫组织化学方法测定B7-H4、P53、CEA在正常胃黏膜、癌前疾病、胃癌中的表达情况。结果 B7-H4在胃癌中高表达,其表达强度与浸润深度、淋巴结转移相关。生存期≤5年的患者B7-H4阳性表达率高于生存期〉5年的患者,差异有统计学意义（P〈0.05）。B7-H4、P53、CEA中两个或两个以上基因异常表达者与无基因或单基因异常表达者相比,在浸润深度、淋巴结转移、组织学分型及病理分期中显著相关。结论 B7-H4与胃癌的发生、发展及预后有着重要关系,联合检测B7-H4、P53、CEA能弥补单项基因检测的不足,有利于胃癌的早期诊断、生物学行为和预后的判断。

Expressions and clinical significance of B7-H4,P53 and CEA in the gastric carcinoma tissues

Objective To study the expression feature and clinical significance of B7-H4,P53 and CEA in the gastric carcinoma tissues and look for immunological indexes to diagnose early gastric cancer.Method Take advantage of immunohistochemistry method to measure the expression of B7-H4,P53 and CEA in normal gastric mucosa,premalignant disease and gastric cancer.Results B7-H4 is highly expressed in gastric cancer,and the expression intension is related to the lymph node metastasis and the infiltration depth.The positive expression rate of B7-H4 of the patient with less than 5-year-life is obviously higher than that of the patient with more than 5-year-life;the difference has statistic significance（P0.05）.Compared with the non gene or there is no unusual expression of single gene,the unusual expression of two or more of genes of B7-H4、P53、CEA has significant collection with invasion depth,lymph node metastasis,organization and pathologic stage.Conclusions There was significant relationship between B7-H4 and the development progress and prognosis of gastric cancer;To inspection B7-H4,P53 and CEA together can compensate the shortage of inspecting single gene,be good for the early diagnosing,judging biological behavior and prognosis to gastric cancer.
