超声判断急性阑尾炎病理类型的价值

目的比较急性阑尾炎不同病理类型的声像图特点,为超声判断急性阑尾炎病理类型提供指导。方法回顾性分析79例手术病理证实为急性阑尾炎患者的超声图像资料,根据不同病理类型,分别总结不同病理类型超声图像特点,比较不同病理类型各超声征像出现率的差异。结果指状低回声肿块、阑尾管腔扩张积液、管腔内强回声伴声影、不规则低回声肿块征像在不同病理类型出现率差异不明显,"靶环征"在单纯性和化脓性阑尾炎之间出现率差异不明显(P0.05),但均高与坏疽性,差异有统计学意义(P0.05);"阑尾周围积液"征像在化脓性和坏疽性阑尾炎之间出现率差异不具有统计学意义(P0.05),但均高于单纯性(P0.05)。结论不同病理类型急性阑尾炎具有一定的特征性超声征像,可为超声检查初步判断急性阑尾炎病理类型提供依据。

Ultrasonic diagnosis of acute appendicitis in the value of pathological types

Objective To compare the ultrasonographic features of acute appendicitis in different pathological types in order to guide the ultrasonic diagnosis of pathological types of acute appendicitis. Methods A retrospective analysis of ultrasonographic data of 79 patients with acute appendicitis which was proved by pathological diagnosis was performed. According to the different pathological types,we sum- marized ultrasound image characteristics of different pathological types, and compared differences of uhrasonography within different path- ological types. Results Significantly distended of the appendix with effusion,higher echo with shadow in the appedndix lumen,irregular lower echo mass were not obviously different in the different pathological types. Incidence rate of ＂target like sign＂ was not significantly different between the simple and phlegmonous appendicitis（ P 〉 0.05 ）, but were higher than that in acute gangrenous appendicitis, the difference was statistically significant（P 〈 0.05 ） ;Incidence rate of ＂effusion around the appendix＂ was not statistically different（P 〉 0. 05） between acute phlegmonous and gangrenous appendicitis, but higher than that of the simple type （ P 〈 0.05 ）. Conclusion The dif- ferent pathological types of acute appendicitis with ultrasound features of certain signs may be useful in preliminary judgment of pathologi- cal types of acute appendicitis.
