C-反应蛋白在老年社区获得性下呼吸道感染中的临床价值

目的　了解 C-反应蛋白 (CRP)在老年社区获得性下呼吸道感染中的临床价值。方法　选择我院2 0 0 2年 1 1月～ 2 0 0 3年 1 0月住院及家庭病床中社区获得性下呼吸道感染老年患者 96例 ,采用免疫比浊法测定治疗前后 CRP水平 ,并与白细胞计数、血沉和体温作比较。结果　治疗前 CRP水平明显升高达 91 .98± 5 4 mg/ L ,其阳性率远高于白细胞计数升高、血沉增快及发热等常规指标。而抗菌治疗一周后又迅速降至接近正常水平 (1 0 .77± 7.5 mg/ L )。结论　 C-反应蛋白水平测定可以作为一种诊断老年社区获得性下呼吸道感染的敏感且可靠的指标 ,也是考核抗菌治疗疗效的一个指标

Clinical values of C-reactive protein on diagnosing acquired lower respiratory infections among elder population

Objective To understand the clinical values of C-reactive protein in diagnosing acquired lower respiratory infections among elder population.Methods Ninety-six cases of community-acquired lower respiratory infection elder inpatients who had been admitted in our hospital between November 2003 and October 2003 were included for the study. The patients' C-reactive protein values were measured by using immunity-turbidity method and were compared between prior to and after the treatment. Also included in the measures were the total account of white blood cells, sedimentation rate, and body temperature. Results Prior to the treatment, the CRP level was high up to 912.98 54mg/L, which was much more apparent than the commonly used parameters such as total account of white blood cells, the increase of sedimentation rate and of body temperature. After one-month treatment using, the CRP quickly returned to the normal level (10.77 7.5mg/L).Conclusion C-reactive protein value can be a sensitive and reliable parameter for the diagnosis of community-acquired lower respiratory infections among elder populations. It can also be used as an indicator for the effectiveness of anti-bacteria treatment
