完全单孔腹腔镜治疗小儿腹股沟斜疝疗效分析

目的 探讨完全单孔腹腔镜手术治疗小儿腹股沟斜疝可行性及安全性.方法 2014年1月至2016年12月,选取542例临床确诊腹股沟斜疝患儿,于脐部放置10 mm trocar,置入单孔带操作孔道的腹腔镜,由操作孔道置入加长持针器,1-0带针丝线由腹壁穿入腹腔,荷包缝合未闭内环口,腹腔内打结.结果 单侧手术时间平均(15.32±2.53) min,双侧手术时间平均(20.59±2.71)min;术中出血量平均(1.61±0.26) mL,其中复发1例,无其它并发症发生.结论 完全单孔腹腔镜疝囊高位结扎术微创,损伤小,复发率低,完全无疤痕,安全可行,值得推广应用.

Complete single-port laparoscopic approache treats inguinal hernia in children

Objective To explore the clinical safety and feasibility of the single-port laparoscopic approaches for the treatment of inguinal hernia in children.Methods The clinical data of 542 children with inguinal hernia undergoing laparoscopic high ligation of hernia sac from January 2014 to December 2016 were analyzed.10 mm trocar was put through umbilicus.A laparoscope with an operating hole was inserted.An needle-holder which was extended in length was inserted through the operating hole.A needle with 1-0 nonabsorbable suture was introduced into the peritoneal cavity through the anterior abdominal wall.The orifice of the hernia sac was closed around the internal inguinal ring,and then intraperitoneal knot-tying was performed.Results Unilateral operation average time was (15.32 ± 2.53) min.Bilateral operation average time was (20.59 ± 2.71)min.Amount of bleeding was (1.61 ± 0.26)mL.One cases were recurrent.No other complications ocurred.Conclusions Complete approache for the treatment of inguinal hernia in children is minimally invasive,little harm,low recurrence rate,no scar.It is safe and feasible.The approach is worth popularization and application.
