摘    要:目的 探讨血清胱抑素C(CysC)与尿微量蛋白系列检测在评价过敏性紫癜(HSP)患儿早期肾功能损害方面的临床意义.方法 选择2012年4月至2013年10月在梅州市嘉应学院医学院附属医院儿科住院及门诊的45例HSP患儿,45例健康儿童为对照组,在常规肾功能、尿常规检查的同时予检测血清胱抑素C、尿微量蛋白系列,对两组所得数据进行统计学分析.结果 HSP组血清CysC、尿微量蛋白系列指标中1项或1项以上异常率为86.67%(39/45),与健康组比较差异有统计学意义(P<0.05).结论 HSP患儿常规肾功能、尿常规检查未出现异常时,部分患儿血清CysC、尿微量蛋白系列已有异常变化,提示比传统肾功能、尿常规检查更早、更敏感地发现肾脏损害.

Abstract:Objective To explore the clinical significance of detection of serum cystatin C (CysC) and urinary trace proteins in the assessment of early impaired renal function induced by allergic purpura (HSP) in children.Methods 45 hospitalized or clinic pediatric patients with HSP who had been treated in our department during the period of April 2012 to October 2013 were selected as study subjects,and 45 healthy children were recruited as a control group.In addition to conventional renal function and urine examinations,serum cystatin C and urinary trace proteins were detected,and the results of all the examinations were statistically analyzed in the two groups.Results As compared with the control group,86.67% of the chilren with HSP (39/45) developed one or more abnormal indexes in the detection of serum CysC and urinary trace proteins,and the difference was statistically significant (P < 0.01).Conclusions Abnormal changes in serum CysC and urinary trace proteins occur in some of the children with allergic purpura even though no abnormality is found in conventional renal function and urine examinations,suggesting that detection of serum CysC and urinary trace proteins is more sensitive in the early identification of kidney damage than traditional kidney function and urine routine examinations.
