机械通气在治疗重度有机磷中毒呼吸衰竭的作用

目的　探讨机械通气在抢救急性有机磷农药中毒所致呼吸衰竭中的作用。方法　对我院收治的重度有机磷农药中毒并发呼吸衰竭 ,按临床特点分为两组 ,采用双向气道正压机械通气的 6 4例患者进行了临床分析。结果　呼吸衰竭脱机成功、痊愈出院 5 9例 (92 19% )。中枢性呼吸衰竭平均上机时间 6 8h ,均为一次脱机。中间综合征平均上机时间 4 9h ,无一例死亡。机械通气前后血气分析相差显著。结论　机械通气是治疗有机磷中毒所致呼吸衰竭的关键环节 ,急救成功与其时机选择密切相关 ,同时重视合理使用足量复能剂 ,迅速达到阿托品化 ,加强气道护理才能有效降低有机磷中毒的死亡率。

Effect of mechanical ventilation on the cases with severe organophosphorus pesticides poisoning

Objective To investigate the effect of mechanical ventilation on the patients with acute organophosphorus pesticide poisoning(AOPP).Methods To analyse all the 64 cases used biphasic intermittent positive airway pressure with respiratory failure as a result of AOPP, and to divide the patients into two groups (A and B) according to clinical characteristics.Results The cured rate of respiratory failure was 92.19%.The time of mechanical ventilation were 6.8, 49 h in two groups respectively. The change of artery blood gas before and after mechanical ventilation was significant differences.Conclusion Mechanical ventilation is a key link in treatment for respiratory failure as a result of AOPP. Meanwhile, emphasis was put on sufficient and reasonable oxime therapy, faster atropinization and airway care is helpful to prevent the development of complication.
