TIL细胞治疗恶性胸(腹)腔积液的临床研究

目的探索CD3单抗和IL-2联合诱导、活化的TIL治疗恶性胸腹水的最佳培养时间和临床疗效.方法抽取患者胸腹水分离TIL,用含rIL-2、CD3单抗的1640完全培养液培养,扩增4周以上,收获细胞局部注入治疗恶性胸腹腔积液,共15例.结果TIL培养至30 d时扩增倍数最高、杀伤活力最强;治疗后完全缓解（CR）9例,占60%,部分缓解（PR）4例,占27%.总有效率87%.结论TIL体外诱导、培养30d,用于治疗恶性胸腹水,效果明显,安全,无毒副作用.

Clinical experiment about treating the malignant pleura fluid and ascites with TIL

Objective To explore the best cultivation time and clinical curative effect of the TIL(tumor infiltrating lymphocytes)for treatment of malignant pleura fluid and ascites.Methods Collecting patient's malignant pleura fluid,ascites and separating TIL then cultured by 1640 culture medium including rIL-2,CD_3 McAb etc.At last the eligible TIL were harvested when increasing more than 4 weeks to treat 15 patients suffered from malignant pleura fluid and ascites.Results TILs increase multiple to be most high and killing activity to be most strong when it has cultured for 30 days,and totally alleviate(CR)after curing is 9 cases accounted for 60%.The partially alleviate(PR)is 4 cases accounted for 27%.Overall efficient rate is 87%.Conclusion TILs led and cultured 30 days outside body to treat malignant pleura fluid and ascites have obvious effect,safe and nonpoisonous.
