AO技术治疗肱骨髁间骨折

目的：探讨ＡＯ技术结合ＣＰＭ功能锻炼治疗肱骨髁间骨折的效果。方法：１５例肱骨髁间骨折按ＡＯ／ＡＳＩＦ分类，Ｃ１３例、Ｃ２７例、Ｃ３５例，均采用经鹰嘴截骨入路切开复位，髁间用螺丝钉固定，肱骨远端内外侧骨柱用双钢板固定，一块重建钢板，一块１／３管型钢板，术后早期ＣＰＭ功能锻炼。结果：平均随访１８年，骨折全部愈合，平均愈合时间４个月，２例并发尺神经炎，无切口感染、肘内翻畸形及骨化性肌炎发生，肘关节功能优良率为９２％。结论：ＡＯ技术结合术后早期ＣＰＭ功能锻炼对治疗肱骨髁间骨折极其有效。

Experience of Treatment of Humeral Intercondylar Fractures by AO Technique

Objective:To evaluate the outcome of humeral intercondylar fracture treated by AO technique combined with CPM exercise.Methods:15 patients with humeral intercondylar fracture were classified according to AO/ASIF,C1 in 3 cases,C2 in 7 cases and C3 in 5 cases.By olecranon osteotomy approach all of them underwent open reduction and internal fixation followed by CPM exercise. Intercondylar parts were fixed by screws.Medial and lateral bone columns of distal humerus were fixed by double plates(a reconstruction plate and a 1/3 curvature tube plate).Results:Followed up period averaged 18 years.All fractures healed with an average duration of 4 months.The elbow function was excellent or good in 92%.2 cases complicated ulnal neuritis and without other complication such as infection,cubital varus or heterotopic ossification.Conclusions:AO technique combined with CPM exercise are most effective method for treatment of humeral intercondylar fracture.
