猪慢性缺血心肌激光血运重建对电生理稳定性的影响

目的 探讨慢性缺血心肌激光血运重建术(TMLR)对电生理稳定性的影响。方法 用Ameroid收缩环制备猪慢性心肌缺血模型,在其右心室心尖部及流出道行心内程序电刺激检测室性心动过速、心室颤动的诱发率,比较激光血运重建组与对照组间的差异。结果 对照组10例中,2例诱发出单形性持续性室性心动过速.2例诱发出多形性室性心动过速持续约10s转为心室颤动,2例诱发出心室颤动,4例未能在刺激终点诱发出室性心动过速、心室颤动,恶性心律失常诱发率60％。治疗组10例中,2例诱发出心室颤动,恶性心律失常诱发率20％,两组间存在显著差异。结论 慢性缺血心肌激光血运重建可能提高电生理稳定性,减少冠心病猝死的发生。

The effect of transmyocardial laser revascularization on electrophysiologic stability of the chronic ischemic myocardium in pigs

Objective To investigate the effect of transmyocardial laser revascularization (TMLR) on electrophysi-ologic stability of the chronic ischemic myocardium in pigs. Methods Chronic coronary stenosis pig model was prepared with Ameroid constrictors, the inducibility of malignant ventricular tachyarrhythmias was compared between the TMLR group and control group with a programmed electrical stimulation protocol. Result In the TMLR group (n=10), ventricular fibrillation (VF) was inducted in 2 pigs(20%),In the control group(n=10), ventricular tachycardia(VT) or VF was inducted in 6 pigs(60%). The difference of the incidence of malignant arrhythmias between the two groups are significant(p<0. 01). Conclusion Transmyocardial laser revascularization improves the electrophysiologic stability of the ventricular chronic ischemic myocardium.
