儿童外周血EB病毒DNA检测的临床意义

摘    要:目的探讨Epstein-Barr病毒(EB病毒)DNA检测的临床应用价值。方法应用荧光定量聚合酶链反应病毒核酸扩增方法,定量检测儿科以发热为主诉的患儿外周血EB病毒DNA载量。阳性病例进行临床分析和抗病毒治疗观察。结果检测522例间断和/或持续发热患儿,阳性(病毒载量≥50拷贝/ml)102例,阳性率19.5%。病毒载量最高4.44×10^6拷贝/ml,最低4.3×10^2拷贝/ml。阳性病例疾病病种较多。部分阳性病例抗病毒治疗显效。结论静脉血EB病毒DNA检测是可行的实验室诊断方法;儿科以发热为主诉的患儿可能有19.5%左右存在EB病毒感染或隐性感染。EB病毒DNA检测阳性患儿应适当抗病毒治疗并随访。

Abstract:Objective To appraise the clinical value of Epstein-Barr virus(EB virus) DNA detection.Methods Fluorescence quantitative polymerase chain reaction-virus nucleic acid amplified method was used to detect the EB virus load in peripheral blood of 522 children with fever,and the positive cases were analyzed and treated with antivirus therapy.Results Of 522 cases with intermittent and/or sustained fever,102 cases were positive(load of EBV-DNA≥50 copies/ml) for EB virus DNA detection,and the positive rate was 19.5%.The maximum load of EBV-DNA was 4.44×106 copies/ml,and the minimum was 4.3×102 copies/ml.The positive cases included many associated diseases,and antivirus therapy was effective to some of them.Conclusions The detection of EB virus DNA in peripheral blood is a practicable laboratory diagnostic method for EB virus infection or latent infection.The rate of EB virus infection or subclinical infection was about 19.5% in the children with fever.All the positive cases should be treated with antivirus therapy and followed up for some time.
