经尿道前列腺电切术后膀胱颈挛缩的防治

目的探讨腔内治疗经尿道前列腺电切术（TuRP）后膀胱颈挛缩（BNC）的疗效。方法回顾性分析TURP32例BNC患者，经尿道置入电切镜切除膀胱颈后唇组织，再用冷刀切开膀胱颈．然后再换用电切刀彻底切除疤痕组织。结果32例患者术后症状明显改善，最大尿流率16．2±4．0ml／s和平均尿流率6．8±2．0ml／s均高于术前（P〈0．05）。术后平均随访28个月，一次性手术治愈28例，复发4例，经再次或三次手术治愈。结论经尿道腔内手术治疗TURP术后BNC是一种安全，疗效确切的微创手术方法。

Treatment and prevention of bladder neck contracture after transurethral resection of prostate

Objective To study the therapeutic efficacy on the contracture of bladder-neck after transurethral resection of prostate （TURP） with endoscopic treatment. Methods From July 2003 to December 2009,32 cases with contracture of bladder-neck after TURP received surgical treatment were analyzed retrospectively. Via urethral indwelling eletroresectoscope,the posterior lip tissues of bladder-neck were resected.And then, the bladder-neck was incised with cryoprobe. Finally, the cicatricial tissue was resected completely with electric knife.Results The symptoms were significantly improved in 32 cases after operation. The maximum uroflow rate was in 16.2 ± 4.0 ml/s and average uroflow rate was in 6.8±2.0 ml/s. The maximum uroflow-rate and average uroflow rate were higher than that before operation （P〈0.05）. The average postoperative follow-up time was 28 months. 28 cases were cured in one-time operation. The symptomatic recurrence was in 4 cases and those cases were cured by 2 or 3 times operation. Conclusion Transurethral intracavitary （endoscopic） operation is a safe and eutherapeutic minimally invasive surgical method in the treatment of contracture of bladder-neck after TURP. BNC after TURP surgery is a and effective method of minimally
