血糖水平与糖尿病代谢性酸中毒的关系分析

摘    要:目的:了解血糖与代谢性酸中毒的关系。方法:对我院近年收治的170例糖尿病患者按标准pH(spH)值大于或小于7.35分为代谢性酸中毒组(A组)和非代谢性酸中毒组(B组),以t检验对两组间血糖(PG)血气指标,肾功能,电解质进行比较,并以实测pH及spH为因变量,其余指标为自变量进行偏相关分析。结果:A组spH,pH,二氧化碳分压(PaCO2),实测碳酸氢根(HCO3^-)及剩余碱(BE)均低于B组,而PG则高于B组,差异有显著性。spH与年龄,pH,PaCo2,HCO3^-,BE呈正相关,与PG,Cl^-,BUN,PaO2呈负相关。pH与年龄,spH,PaCO2,HCO3^-,BE呈正相关,与PG,K^-,Cl^-,BUN及PaO2呈负相关。在除外其他指标影响下pH仍分别在K^ ,Cl^-独立相关;spH与K^ ,PG独立相关。结论:高血糖是代谢性酸中毒的独立危险因素。

Abstract:Objective:To analyzed the relationship between blood glucose level and metabolic acidosis.Methods:170 cases were divided into two groups:the metabolic acidosis group (A group) and non metabolic acidosis group(B group).The blood glucose,blood oxygen indicators,kidney function and electrolyte were detected and statistically analyzed.Results:A group showed the lower spH,pH,PaCO 2,HCO - 3 and BE as compared with that of B group.However,the blood glucose level in A group was significantly higher than that of B group.The spH showed the positive correlativity to the age,pH,PaCO 2,HCO - 3 and BE,but the negative correlativity to blood glucose,Cl -,BUN and PaO 2.The pH showed the positive correlativity to the age,spH,PaCO 2,HCO - 3 and BE,but negative to blood glucose,K +,Cl - and PaO 2.There was a independent correlativity between pH and K + and Cl -.The independent correlativity between spH and K +,blood glucose was also found.Conclusion:The hyperglycemia is the independent risk factor of metabolic acidosis.
