老年消化性溃疡临床特点

目的探讨老年消化性溃疡的临床特点。方法通过老年组62例和中青年组108例患者,临床症状、胃镜检查和幽门螺杆菌(HP)试验及治疗半年后复查对比。结果老年组胃溃疡35例(56.5%),十二指肠溃疡8例(12.9%),复合溃疡19例(30.6%),HP阳性42例(67.7%);中青年组胃溃疡20例(18.5%),十二指肠溃疡72例(66.7%),复合溃疡16例(14.8%),HP阳性92例(85%)。治疗半年后复查,老年组48例(77.4%)溃疡愈合,4例癌变(6.5%);中青年组102例(94.5%)溃疡愈合,1例(0.9%)癌变。结论老年消化性溃疡有以下特点:临床缺乏典型症状;复合溃疡发病率高;癌变率高;HP感染并非随年龄增大而升高;常伴有精神因素。

Clinical characteristics of peptic ulcer for the eldly

Objective To investigate the clinical characteristics of peptic ulcer for aged patients.Methods We retrospectively analyzed the clinical characteristics,the clinical data of endoscopy and HP test of 108 young and middle-aged patients and 62 elderly patients. Results In elderly group,gastric ulcer were 35(56.5%),duodenal ulcer were 8(12.9%),complex ulcer were 19(30.6%),HP( )were 42(67.7%).In young-middle-aged group,gastric ulcer were 20(18.5%),duodenal ulcer were 72(66.7%),complex ulcer were 16(14.8%),HP( ) were 92(85%).To recheck after treating half-year,in elderly group,ulcer-healing were 48(77.4%),ulcer-canceration were 4(6.5%);in young-middle-aged group,ulcer-healing were 102(94.5%),ulcer-canceration was 1 case(0.9%). Conclusion The peptic ulcer for aged patients had some symptoms as follows:the lack of typical symptom in clinic;the higher incidence of complex ulcer;the higher ulcer-canceration rate;the HP infection not to be increased along with age,the frequent on comitance psych-factor.
