肺淋巴管肌瘤病的螺旋CT诊断

目的探讨肺淋巴管肌瘤病例的临床特点及螺旋CT表现,提高对该病的认识及诊断水平。方法回顾分析7例经肺组织活检病理证实的肺淋巴管肌瘤病(plam)并行螺旋CT检查的患者资料,分析其临床表现、X线胸片及螺旋CT特点。结果本组7例病例临床表现均以呼吸困难为主,伴不同程度的胸闷,有2例伴有反复性气胸。肺功能以阻塞性通气功能障碍为主的混合性通气功能障碍;X线胸片示:有7例两肺均呈弥漫性的网状阴影,3例有大小不等的蜂窝状囊腔,4例伴肺气肿,2例伴有胸膜肥厚。螺旋CT平扫示:均见散在大小不等的薄壁囊腔,其壁厚1.3～2.0 mm之间,<2.0 mm部分囊腔见小血管影,囊腔之间可见正常肺组织;螺旋CT则能显示清楚以上的表现。结论螺旋CT能全面的显示plam特征性表现,对该病的诊断鉴别有重要价值。
