Barrett食管及其腺癌

报道１２例Ｂａｒｒｅｔｔ食管及其腺癌，其中８例癌完全位于食管内，４例大部分位于食管，部分累及胃贲门。７例癌周食管粘膜见有良性状上皮，其中作了大量取材的５例有特殊型、交界型和胃阍型上皮各７５．９％、２１．８％和２．３％。３种类型上皮未显带状分布，而是在交界型上皮的基础上发生斑片状脾性化。分别有５例和３例见有残余的鳞状上皮岛和食管腺。

Barrett esophagus and its adenocarcinoma

Twelve cases with Barrett esophagus and associated adenocarcinoma were studied.The tumor mass in eight cases were located entirely within the esophagus, four cases were noted to involve the gastric cardia but the better part in the esophagus. The benign Barrett's epithelium (BE) was observed in the esophagus around tumors of seven cases, five of these, a specialized type epithelium account for 75.9%, a junctional type 21.8%, and a gastric fundic type 2.3%,respectively. Zonation of the three epithelial type may not be well defined in all cases, while patches of intestinal metaplasia occured on the junctional type epithelium. Multiple residual squamous islands and esophageal submucosal glands were found within the regions of BE in five and three cases respectively. In four cases, dysplasia has been observed in six areas of BE adjacent to the tumors, 4 of these occured in the specialized type epithelium. Transformable change of dysplasia to main carcinoma were found in two cases. The clinicopathologic features and diagnosis of the Barrett's esophagus and its adenocarcinoma were discussed.
