非脱垂病变子宫阴式与腹式切除术比较

目的探讨阴式切除非脱垂病变子宫手术在妇科领域的应用价值。方法回顾性分析比较阴式及腹式切除非脱垂病变子宫手术286例，记录患者的术前阳性体征、术中发现及术后恢复情况。结果阴式手术组手术时间及术中出血量明显少于腹式手术组，分别为（72．6±18．3）min和（96．2±15．3）min、（88．6±34．3）mL和（105．6±25．0）mL，P均〈0．05；静脉用抗生素时间、肛门排气时间及住院时间均明显短于剖腹手术组，分别为（3．2±1.6）d和（5．2±2．8）d、（18±9）h和（31士9）h、（4．8±1.5）d和（7．8±1.6）d，P均〈0．05。结论 阴式切除非脱垂病变子宫手术具有手术时间短、术中出血量少、术后胃肠功能恢复迅速、住院时间短等优点，值得推广应用。

Comparison of Effect of Transvaginal Hysterectomy and Laparotomy Hysterectomy for Non-prolapsed Uterus

Objective To investigate moderate enlarged normal-positioned uterus. Methods The positive physical sign, detection in operation, length of stay and were recorded, and the postoperative recovery in 286 patients underwent transvaginal hysterectomy or transabdominal hysterectomy were observed. Results Operative time and bleeding in operation in the transvaginal hysterectomy group were fewer than in abdominal group （72.6±18.3）min vs （96.2±15.3）min,（88.6±34.3）mL vs （105.6±25.0）mL, P〈0.05]. The antibiotic time,functional recover time of bowel and length of stay in hospital of the vaginal group were shorter than those in the abdominal group （3.2±1.6）day vs （5.2±2.8） day;（18±9） hour vs （31±9）hour;（4.8±1.5） day vs （7.8±1.6） day,P〈0.05]. Conclusion The vaginal hysterectomy would be better than the abdominal hysterectomy in operativing time, bleeding in operation,functional recover of bowel and the length of stay. It should be worth of popularizing, and using.
