2种内固定法治疗老年股骨粗隆间骨折疗效对比观察

目的比较2种内固定方法治疗老年股骨粗隆间骨折的临床疗效。方法将47例股骨粗隆间骨折患者随机分为股骨近端防旋髓内钉（PFNA）组26例和动力髋螺钉（DHS）组21例。PFNA组采用PFNA内固定治疗，DHS组采用DHS内固定治疗。观察2组手术效果及手术时间、术中出血量、术后下床负重时问等手术情况。结果2组手术效果比较差异无统计学意义（P〉0．05）；PFNA组手术时间、术后下床负重时间短于DHS组，出血量少于DHS组，差异均有统计学意义（P〈0．05）。结论PFNA和DHS治疗老年股骨粗隆间骨折均能获得较好疗效，与DHS内固定相比，PFNA具有手术时间短、固定牢固、术中损伤小、术中出血少、术后负重时间早等优点，是治疗股骨粗隆间骨折的理想内固定物。

Comparative effect observation of two kinds of fixation in the treatment of intertrochanteric fractures in elderly

Objective To compare the clinical effect of two kinds of internal fixation methods in the treatment of in- tertrochanteric fractures in elderly. Methods 47 cases patients with intertrochanteric fracture were randomly divided into proximal femoral nail anti-rotation （PFNA） group 26 cases and dynamic hip screw （DHS） group 21 cases. PFNA group were treated with PFNA fixation, DHS group were given DHS internal fixation. Observed operative effect and operative time, blood loss,postoperative ambulation time of two groups. Results Operative effect of two groups showed no significant difference （P 〉 0. 05 ） ;operative time, postoperative ambulation time of PFNA group were shorter than those of DHS group, blood loss of PF- NA group was less than that of DHS group, the differences were statistically significant （ P 〈 0.05 ）. Conclusion PFNA and DHS treatment of intertrochanteric fractures in elderly can get better effect, compared with DHS, PFNA has a shorter operative time, fixation, surgery less damage,less blood loss, postoperative weight earlier, and it＇s ideal for the treatment of intertrochan-teric fracture fixation.
