肺心病人肺型P波与主肺动脉内径变化的研究

目的探讨肺心病人心电图的肺型P波与超声心动图的主肺动脉内径变化的关系。方法收集肺心病伴肺型P波的临床资料45例为肺心病组;并按超声心动图诊断分为右心扩大组26例、无心腔扩大组19例;在我院健康检查者30例为对照组;对其主肺动脉内径与肺型P波的变化进行比较。结果肺心病组主肺动脉内径较对照组显著增宽(P<0.01);肺心病右心扩大组主肺动脉内径较无心腔扩大组显著增宽(P<0.001);无心腔扩大组肺型P波较右心扩大组显著增高(P<0.05)。结论心电图肺型P波是诊断肺心病人心脏未扩大时的敏感指标,且具有仪器携带方便、价格便宜等特点。超声心动图对肺心病右心增大时可提供更为可靠的依据。

Changes of Pulmonary P - wave and Main Pulmonary Artery Diameter in Patients With Pulmonary Heart Disease

Objective To understand the association of changes of pulmonary P-wave(PPV) presented on ECG and main pulmonary artery diameter(MPAD) measured by ultrasono-cardiogram(UCG) in patients with pulmonary heart disease(PHD).Methods Data were acquired from 45 patients with PHD examined by ECG and UCG to analyze PPV and MPAD.According to the UCG diagnosis,all PHD patients were assigned into the groups with(26) or without(19) the right ventricular enlargement as compared with normal volunteers(30) identified by health examination.Results As compared with the normal volunteers,patients with PHD had significantly increased MPAD(P<0.01);PHD patients with right ventricular enlargement presented higher width of MPAD than patients without MPAD(P<0.001);Peak of PPV in PHD patients without heart enlargement was measured higher than the group of patients with right ventricular enlargement(P<0.05).Conclusion The changes of PPV on ECG are sensitive and could be applied in diagnosis of PHD without enlargement of right ventricle,moreover,the examination is economical and convenient. Using of UCG to determine MPAD may provide more reliable references in diagnostic work-up of PHD patients with enlarged right ventricle.
