| 摘 要： | 采用ＰＲＡ ＳＴＡＴ技术筛选肾移植受者特异性抗ＨＬＡ抗体（ｓＨＬＡ ＩｇＧ）。将１２８份血清分成４组，包括首次移植术前及术后一个月、肾功能稳定期和移植肾失功再血透者。按ＰＲＡ ＳＴＡＴ酶联检测试剂盒要求操作。抗体特异性的确定根据阳性结果的反应格局综合判断。结果表明所有样本的测定结果其有效性均符合试剂盒要求。本法操作简捷、耗时４ｈ；结果重复性好、批内与批间差异（ＣＶ）均＜５％；ＰＲＡ ＳＴＡＴ百分比＜８０％的致敏者其抗体特异性可以确定。临床应用显示：再次移植者致敏率高达７５％，显著高于首次移植者（１８８％）；术前及术后ｓＨＬＡ ＩｇＧ抗体的监测对移植效果具有预测和指导意义。

| Abstract: | Atotalof128serumsamplesweretestedinfourgroupsofrenalallograftrecipientsofonemonthbeforeandaftertransplantations,612monthsposttransplantationwithwellrenalfunctionsandthetransplantationcandidateswaitingforasecondorthirdgraftThepanelreactiveantiHLAIgGantibodiesweredetectedfromserabymeansofPRASTATELISAkitinthesesera,andthevalidateassaywasconcordantwiththiskitin128serumsamplesTheoveralltimeoftheassaywasonly4hours,withthewithinrunvariationrangedfrom234to317%andthebetween dayvariationrangedfrom294to386respectivelyAstoclinicaltrials,itappearedthat75%ofthesecondorthirdtransplantationcandidateshadacurrentPRASTATabove10%,bycontrast,only188%ofthecandidatesforthefirstgraftwerehighlysensitizedThismethodoftestingisarapid,highlyreproducible,objectiveandreliableperformance,suitableforclinicaluseinorgantransplantation. |
