后腹腔镜下手术与开放手术对肾癌患者白细胞介素水平以及HPA轴活性影响的比较

目的：研究后腹腔镜下手术与开放手术对肾癌患者白细胞介素水平以及HPA轴活性的影响。方法：选取在本院接受肾癌根治术的患者纳入研究,根据手术方式不同分为接受后腹腔镜下手术的观察组和接受开放手术的对照组,比较两组的白细胞介素-1（IL-1）、白细胞介素-6（IL-6）以及HPA轴相关激素促肾上腺皮质激素（ACTH）、皮质醇（Cor）、肾上腺素（E）、去甲肾上腺素（NE）水平。结果：观察组的ACTH、Cor、E、NE、IL-1、IL-6水平均低于对照组,差异有统计学意义（P〈0.05）;且IL-1、IL-6水平与ACTH、Cor、NE、E水平呈正相关。结论：后腹腔镜下手术有助于降低白细胞介素水平以及HPA轴活性、缓解手术创伤所造成的炎症应激反应,是治疗肾癌更为理想的手术方式。

Comparative study on the effect retroperitoneal laparoscopic operation and open operation on interleukin levels and HPA axis activity of patients with renal cell carcinoma

Objective： To study the effect of retroperitoneal laparoscopic operation and open operation on interleukin lev- els and HPA axis activity of patients with renal cell carcinoma. Methods.. Patients with renal cell carcinoma received radical ne- phrectomy in our hospital were enrolled and divided into observation group （given retroperitoneal laparoscopic operation） and control group （given open operation）. Then interleukin-2, interleukin-6 and HPA axis hormone adrenocorticotropic hormone, cortisol, epinephrine, norepinephrine levels were compared. Results： ACTH, cortisol, epinephrine, norepinephrine, interleu- kin-l, interleukin-6 levels of observation group were lower than those of control group; IL-1, IL-6 levels were positively corre- lated with ACTH, Cor, NE, E level. Conclusion： Retroperitoneal laparoscopic operation is helpful to reduce the levels of inter- leukin and HPA axis activity, alleviate inflammatory and stress reaction caused by operation trauma; it＇s an ideal surgery ap- proach in treating renal cell carcinoma
