不同修补方法在紧张部小穿孔中的效果比较

目的探讨不同修补方法在紧张部小穿}L中的效果。方法选取2005年12月～2010年3月于笔者所在医院进行治疗的54例鼓膜紧张部小穿孔患者为研究对象，将其随机分为A组（烧灼治疗组）27例和B组（耳内镜下脂肪移植修补组）27例，对两组患者的治疗总有效率、听力改善情况、愈合时间及患者满意情况和治疗前后的血清总IgE、IL-5、IL-6、INF-γ及SOD水平进行比较。结果B组的治疗总有效率高于A组，听力改善情况优于A组，愈合时间短于A组，患者总满意率高于A组，而治疗后B组血清总IgE、IL-5、IL-6、INF-γ及SOD水平均低于A组，有显著性差异（P〈0．05）或有非常显著性差异（P〈0．01）。结论耳内镜下脂肪移植修补术在紧张部小穿孑L的治疗中优势明显，且对患者的不良影响小，值得临床推广应用。

The comparison of the effects of different repair methods in small perforation of membrana tensa

Objective To study and compare the effects of different repair methods in small perforation of membrana tensa. Methods 54 patients with small perforation of membrana tensa in our hospital from December 2005 to March 2010 were selected as research object, and they were randomly divided into group A （ cauterization treatment group） 27 cases and group B（autologous fat transplantation under otic endoscope in myringoplasty group）27 cases, then the total effective rate of treatment, hearing improvement, healing time, satisfaction rate and serum total IgE, IL-5、IL-6、INF-γ and SOD of the two groups before and after the treatment were detected and compared. Results The total effective rate of treatment of group B was higher than that of group A, hearing improvement was better than that of group A, healing time was shorter than that of group A, satisfaction rate was higher than that of group A, but the serum total IgE, IL-5、IL-6、INF-γ and SOD of group B after the treatment were lower than those of group A, there were significant differences or there were very significant differences （ P 〈 0. 05 or P 〈 0. 01 ）. Conclusion The advantages of Autologous fat transplantation under otic endoscope In myringoplasty in small perforation of membrana tensa is obvious, and the harmful effects is smaller for the patients, and it is worthy of popularization and application.
