坎地沙坦联合胺碘酮治疗心房颤动的临床观察

目的 观察口服胺碘酮联合血管紧张素Ⅱ受体拮抗剂（ARB）坎地沙坦治疗心房颤动的疗效,及对左心房功能的影响.方法 入选62例持续性心房颤动患者,随机分为两组,单纯口服胺碘酮（观察组）31例,口服胺碘酮＋坎地沙坦（对照组）31例,疗程观察18个月.结果 共61例完成治疗,随访18个月,两组患者治疗12～18个月后左心房内径观察组显著高于对照组（P〈0.05）;治疗后对照组窦性心律维持12～18个月的均明显高于观察组（P〈0.05）.结论 口服胺碘酮联合坎地沙坦可逆转左心房扩大,比单用胺碘酮能更有效地转复房颤,维持窦性心律.

Effect of candesartan combined with amiodarone treating atrial fibrillation

Objective To investigate the efficacy of amiodarone combined angiotensin 11 receptor antagonist （ARB） candesartan treating atrial fibrillation, and left atrial function. Methods A total of 78 patients with persistent atrial fibrillation were randomly divided into two groups： simple amiodarone group （Observed group, 31 cases）, amiodarone ＋ candesartan group （compared Group, 31cases） ,they were treated for 18 months,following up for 18 months. Results Totally 61 cases completed the treatment. The left atrium in simple amiodarone group was significantly higher than that in amiodarone ＋ candesartan group 12 - 18 months after treated （P 〈0.05） ;the maintenance rates of 12 months,18 months of sinus rhythm in amiodarone ＋ candesartan group were significantly higher than those in simple amiodarone group （ P 〈 0. 05 ）. Conclusion Amiodarone combined candesartan can reverse expansion of left atrium, and are more effective for transfer and maintain sinus rhythm compared with simple amiodarone treatment in patients with atrial fibrillation.
