2型糖尿病肾病患者危险因素分析

目的探讨2型糖尿病肾病的I临床特点及危险因素，为防治糖尿病肾病提供依据。方法选取2型糖尿病患者208例，分为两组，即糖尿病肾病组（98例）和非糖尿病肾病组（110例），于入院时询问患者病史（糖尿病家族史、高血压、高血脂及冠心病）；于禁食12h后，抽取静脉血测其空腹血糖（FPG）、三酰甘油（TG）、总胆固醇（TC）、高／低密度脂蛋白（HDL—C／LDL—C）；另外监测餐后2h血糖（2hPG）及糖化血红蛋白（HbAlc）。对其临床资料进行统计分析。结果合并有高血压、高血脂、冠心病及存在糖尿病家族病史的患者患糖尿病肾病的几率较大，两组比较差异具有统计学意义（P〈0．05）；与非糖尿病肾病组比较，糖尿病肾病患者的病程、SBP、DPB、FPG、2hPG、HbAlc及血脂指标（TG、TC、HDL—C、LDL—C）存在较大差异，其差异具有统计学意义（P〈0．05）。结论糖尿病肾病的危险因素与遗传因素、环境因素、糖尿病病程、高血压、高血脂及冠心病等有关。

Clinical feature and risk factors of type 2 diabetic nephropathy

Objective To discuss the clinic feature and risk factors of type 2 diabetic nephropatby, and to provide bases for diagnose and treat of diabetic nephropathy. Methods 208 type 2 diabetics were randomly divided into diabetic nephropathy group （98 cases）and no diabetic nephropathy group （110 cases）. The doctors asked about patient medical history （Family history of diabetes mellitus, hypertension, hyperlipidemia and coronary heart disease ）. After 12 h of nesteia, FPG, TG ,TC ,HDL - C/LDL - C ,2 h PG and HbAlc were analyzed in peripheral vein blood. Results The patients of family histo- ry of diabetes mellitus, hypertension, byperlipidemia and coronary heart disease had higher prevalence of diabetic nephropathy of type 2 diabetes. Compared with no diabetic nephropathy group , the patients of course of disease,SBP、DPB ,FPG,2 h PG, HbAlc ,TG,TC ,HDL- C ,LDL- C had the difference in diabetic nephropathy group. The two groups had a significant difference（ P 〈 0. 05 ）. Conclusion The risk factors of diabetic nephropathy are related to genetic factors, course of diaease, hy- pertension, hyperlipidemia and coronary heart disease.
