深部热疗加同步放化疗治疗Ⅲb期宫颈癌临床价值

目的探讨盆腹腔深部热疗联合同步放化疗治疗Ⅲb期宫颈癌的疗效和安全性。方法对2005年11月至2006年10月广西医科大学肿瘤医院70例首次治疗的Ⅲb期宫颈癌患者随机分为观察组和对照组,各35例。两组均进行常规放疗,外照射1周后同步行化疗,DDP 40mg/m2,1次/周,共6周期。另观察组于化疗当天行盆腹腔深部热疗,每次60min,1次/周,共4次。结果观察组完全缓解率、有效率、局部控制率、外照射结束时局部肿瘤完全消退率、1年期无瘤存活率分别为80.00%、97.14%、88.57%、62.86%、88.57%,明显高于对照组(P<0.05)。两组的1年存活率差异无统计学意义(P>0.05)。毒副反应主要为胃肠道反应和骨髓抑制,两组各项早期毒副反应差异无统计学意义(P>0.05)。结论盆腹腔深部热疗加同步放化疗可以提高患者近期疗效,且无严重的并发症,是中晚期宫颈癌治疗的一种安全有效模式,具有临床推广价值。

Clinical study on deep hyperthermia combined with concurrent radiochemotherapy for the treatment of stage Ⅲb cervical cancels

Objective To study the efficacy and safety of deep hyperthermia combined with concurrent radiochemotherapy in the treatment of stage IIIb cervical cancer.Method The 70 cases of cervical cancer patients of stage IIIb were randomly divided into experimental and control groups,each of 35 cases.Conventional external irradiation and intracavitary irradiation were carried out,then followed by concurrent chemotherapy after one week of radiotherapy,administering DDP40mg/m2/week,six cycles totally.The deep hyperthermia with concurrent chemotherapy was given to the experimental group,lasting for 1 hour once weekly,4 times totally.Results The experimental group's complete remission rate,efficiency rate,local control rate,local tumor complete regression rate after external irradiation and 1-year disease-free survival rate were 80.00%,97.14%,88.57%,62.86% and 88.57% respectively,significantly higher than that of in the control group(P<0.05).There was no statistical significance in one-year survival rate between two groups(P>0.05).The toxicity of the two groups mainly were gastrointestinal reactions and bone marrow depression.No statistical correlation was observed in the early toxicity between two groups(P>0.05).Conclusion Deep hyperthermia combined with concurrent radiochemotherapy can improve the recent efficacy,had no serious complication.It was a safe and effective mode to treat advanced cervical cancer and had great clinical value.
