三维适形放疗同期替吉奥化疗治疗术后复发直肠癌的疗效研究

目的探索两种放疗方法并同步替吉奥化疗治疗直肠癌术后复发的治疗反应情况和毒副作用。方法80例符合条件的患者随机分组，观察组40例行三维适形放疗技术，对照组40例采用常规放射治疗，两组靶区总量、分割方式标准及同期化疗方案均相同。结果两组在恶心呕吐、血红蛋白下降、中性粒细胞下降等差异均无统计学意义；观察组IIr级腹泻发生率为7．5％低于对照组27．5％，观察组与对照组1、2、3年的生存率分别为70％、56％、33．3％和65％、48％、23．3％；观察组与对照组1、2、3年的局控率分别为87％、71.1％、60％和72．5％、63％、48％，差异均有统计学意义（P〈0．05）。结论结论三维适形放疗同步替吉奥化疗治疗直肠癌术后复发的疗效确切，局部控制率高，毒副作用可耐受，能明显改善患者生活质量。

THE CURATIVE EFFECT OF THE DIMENSIONAL CONFORMAL RADIOTHERAPY AND THE THERAPY WITH TEGAFUR GIMERACIL OTERACIL POTASSIUM ON RECURRED ECTAL CANCER AFTER CHEMOTHERAPY

Objective To explore the therapeutic reaction and toxic and side effect of postoperative recur fence of rectal cancer when treated with two different therapies as well as tegafur gimeracil oteracil potassi urn. Methods Of the 80 cases, 40 cases in the observation group underwent three dimensional conformal radiation therapy. 40 cases in the control group received routine radiation therapy. The target volume, the standard of dividing methods and homochronous chemo- therapy were same between the groups. Results There existed no significant difference in nausea and vomiting,the decrease in the number of hemoglobin or neutrophil. The incidence of diarrhea at Stage III was 7.5 %, much lower than that （27.5%） in the control group. The survival rates of the observation group and the control group in the 1st, 2nd and 3rd year were 70%, 56% and 33.3% and 65%, 48% and 23.3% respec- tively. The local control rates of the observation group and the control group in the 1st, 2nd and 3rd year were 87%, 71.1% and 60% and 72.5%, 63%band 48% respectively（p〈0.05）. Conclusion The three di- mensional conformal radiotherapy and the therapy with tegafur gimeracil oteracil potassium can produce good curative effects with high local control rate and good tolerance for toxic and side effects to obviously promote patients＇ quality of life.
