硝普钠对老年高血压急症疗效及血液流变学影响观察

目的观察硝普钠对老年高血压急症疗效及血液流变学影响。方法老年高血压急症59例,随机分为硝普钠组(31例)与拉贝洛尔组(28例)。2组均静脉持续用药6h。每例于治疗开始与结束时进行血液流变学检测。结果降压总有效率硝普钠组为100%(31/31),拉贝洛尔组92.9%(26/28),2组差异无显著性(P>0.05),但硝普钠组起效快于拉贝洛尔组。患者经硝普钠治疗后,血液流变学示低切变率下全血黏度,高切变率下全血黏度、血浆比黏度、红细胞聚集指数及血小板聚集率均显著下降(P<0.01或P<0.05),与拉贝洛尔组比较,差异亦有显著性(P<0.05)。拉贝洛尔组血液流变学各指标值虽有下降,但差异无显著性(P>0.05)。与药物有关的不良反应,硝普钠组明显低于拉贝洛尔组(3.2%和21.4%,P<0.01)。结论硝普钠治疗老年高血压急症疗效确切,起效快,不良反应少,且该药有改善血液流变性作用。

Effects of intravenous sodium nitroprusside on hypertensive emergencies and hemorrheology in the aged

Objective To observe effects of intravenous sodium nitroprusside(NP) on hypertensive emergencies and hemorrheology in the aged. Methods Fifty-nine aged cases of hypertensive emergencies were randomly divided into NP group(31 cases) and labetalol(Lab) group (28 cases). The NP group was treated with intravenous NP, and the Lab group was treated with intravenous Lab. Constant intravenous infusion was maintained for 6 hours in both groups. The parameters of hemorrheology were measured before and after treatment. Results The clinical total effective rate in NP group was 100% and 92.9% in Lab group respectively. No significant difference was observed between 2 groups(P>0.05). However, the response in NP group was quicker than that in Lab group. There were significantly few adverse reactions related to the drug in NP group than in Lab group(3.2%vs 21.4%, P<0.01). Conclusions NP can improve hemorrheology in aged patients with hypertensive emergencies. Intravenous NP is a useful treatment for hypertensive emergencies with quicker response and fewer side effects.
