氯吡格雷低反应性与CYP2C19基因多态性分布的研究

摘    要:氯吡格雷是临床常用的抗血小板药物,在体内需通过CYP450酶的转变而发挥其抑制血小板聚集的作用,能有效降低动脉内及术后支架内血栓形成的风险.由于细胞色素P450酶系中起关键作用的CYP2C19酶存在基因多态性,使部分患者产生氯吡格雷低反应性即氯吡格雷抵抗.血小板聚集率高,抗血小板作用减弱,增加了不良心血管事件发生率.本文通过对氯毗格雷低反应的患者进行基因多态性分析,指导个体化用药.

Abstract:Clopidogrel is a commonly used antiplatelet drug in clinical, which exerts its inhibitory effect on platelet aggregation in vivo through the transformation of CYP450 enzyme, and it can effectively reduce the risk of thrombosis of artery and stent after operation. Because CYP2C19 enzyme playing the main role of cytochrome P450 enzymes has gene polymorphism, so it makes some patients have low reactivity to clopidogrel as clopidogrel resistance. High platelet aggregation rate, which makes antiplatelet effection weakened, increases the incidence of adverse cardiovascular events. This article through to clopidogrel patients with low response for gene polymor- phism analysis, to guide individualized medication.
