颅内多发动脉瘤的血管内介入治疗

摘    要:目的 探讨血管栓塞术治疗颅内多发性动脉瘤的诊疗经验.方法 对26例颅内多发性动脉瘤(60个动脉瘤)病人的临床资料进行回顾性分析,全部行3D-DSA检查并行血管内介入治疗.结果 术中无动脉瘤破裂,术后病人住院天数10~20日.术后疗效依照GOS评分为优良19例,轻残4例,重残2例,死亡1例,因动脉瘤夹闭术后继发同侧大片脑梗塞死亡.术后6个月,23例遵医嘱行DSA复查均未见动脉瘤复发及新生动脉瘤,2例病人未随访.结论 3D-DSA检查可有效检出颅内多发性动脉瘤,血管内介入治疗颅内多发性动脉瘤安全可行,值得临床推广使用.

Abstract:Objective To summarize intravascular interventional diagnosis and treatment experience for in- tracranial multiple aneurysms. Methods The clinical data of 26 patients with intracranial multiple aneurysms (60 aneurysms) was analyzed retrospectively, which carried out 3D- DSA examination a endovascular interven- tional treatment. Results No aneurysm ruptured during operation, postoperative hospitalization days were 10 20 days. According to GOS scores, 19 cases had excellent postoperative curative effect, mild disability for 4 cas- es, severe disability for 2 cases, 1 case died of the secondary ipsilateral large cerebral infarction. After six months, there were no recurrence of the aneurysm in 23 cases, 2 cases did not follow up. Conclusion 3D - DSA examination can effectively detect intracranial multiple aneurysms, intravascular interventional treatment of in- tracranial multiple aneurysms is safety and feasible.
