玻璃体切除术后白内障超声乳化术

目的 探讨玻璃体切除术后白内障超声乳化术的临床疗效。方法 对41人43眼玻璃体切除术后白内障实行超声乳化手术．手术中单纯行白内障超声乳化9眼。白内障超声乳化I期植入后房型人工晶体15眼，前房型人工晶体4眼。白内障超声乳化联合再次玻璃体切除、硅油填充术8眼。白内障超声乳化联合再次玻璃体切除、惰性气体充填术3眼。白内障超声乳化、人工晶体植入联合小梁切除术1眼。白内障超声乳化、玻璃体腔硅油取出术3眼。结果玻璃体切除术后白内障手术难度大，前房深度不稳定．术后最佳矫正视力较术前视力有提高37眼（声6．04％），视力未提高4眼（9．30％），视力下降2眼（4．65％）。结论 玻璃体切除术后白内障采用超声乳化术，减少了并发症，保留了晶状体后囊膜，并可I期或Ⅱ期植入人工晶体，是玻璃体切除术后白内障摘除的较好的手术方式。

Phacoemulsification in post-vitrectomy eyes

Objective To discuss the clinical characteristics of phacoemulsification in post-vitrectomy eyes.Methods Phacoemulsification was performed in 43 eyes of 41 persons.Nine eyes were undergone phacoemulsification only during cataract surgery.Posterior chamber artificial lenses were implanted into 15 eyes and anterior chamber artificial lenses into 4 eyes during the stage I of phacoemulsification.Phacoemulsification,in combination with vitrectomy and silicone oil tamponade,was performed in 8 eyes,and in combination with re-vitrectomy and inert gas tamponade in 3 eyes.The combined phacoemulsification,artificial lens implantation and trabeculectomy was performed in 1 eye.The combined phacoemulsification and silicone oil removal from vitreous cavity were performed in 3 eyes.Results It was hard to do cataract surgery after vitrectomy and the depth of anterior chamber was instable.After the surgery,the best corrected visual acuity was improved in 37 eyes(86.O4%),unchanged in 4 eyes(9.30%) and worse in 2 eyes(4.65%).Conclusion Phacoemulsification in post-vitrectomy eyes can reduce complications and remain posterior capsular.It is the best way to extract cataract in eyes after vitrectomy,if artificial lens is implanted during the stage I or II.
