重症肌无力胸腺切除术后机械通气的探讨

目的:探讨重症肌无力胸腺切除术后机械通气的危险因素.方法:1987年1月至2003年8月,我科共收治重症肌无力患才42例,运用统计软件对年龄、性别、病程、临床分型、病理类型、手术时间、术前吡啶斯的明用量、术前激素的使用等因素进行多因素logistic回归模型分析.结果:影响机械通气的主要危险因素有:临床分型、病理类型,且呈正相关.结论:重症肌无力胸腺切除术后临床分型、病理类型等因素可作为是否延长机械通气的选择指征.

Exploration on mechanical ventilation in patients with myasthenia gravis after thymectomy

Objective:To explore the risk factors in mechanical ventilation on patients with myasthenia gravis after thymectomy.Methods:42 patients with myasthenia gravis were treated by thymectomy from January 1987 to August 2003.Using logistic regression model,a multi-variate analysis was made with regard to age,sex,disease history,Osserman classication,pathological types,the consumed time of operation,the preoperatie dose of pyridostigmine,the preoperative usage of hormone.Results:The major risk factors that affected mechanical ventilation on patients with myasthenia gravis after thymectomy were Osserman classication and pathological types.Conclusion:Osserman classication and pathologic types might serve as indices for the prolonged mechanical ventilation.
