| 摘 要： | 借助制度经济学理论阐述了新型农村合作医疗制度设计的7个重要"手柄"和典型运行模式,并从政府支持(含保险组织方)、农村居民和医疗服务供给方3个维度,运用ROCCIPI技术方法系统分析该制度实施6年来所取得的成效,识别存在的主要问题及原因,在此基础上提出确保我国新型农村合作医疗制度可持续发展的几个关键改进策略:明确各级政府筹资责任与分担机制;提升基层政府的组织管理技术和经办能力;适度强制或半强制农村居民参加,减少逆选择;适当增加农民个人筹资标准,提高制度保障水平;逐步将门诊基本医疗服务纳入补偿范围,兼顾人群受益面与受益程度;完善对定点医疗服务机构的激励与制约机制,降低供方道德风险;努力培育区域社会资本,增强农民信任感。
