前S抗体与乙型肝炎病毒清除及临床预后的关系

为了探讨前S1和前S2抗体的临床意义,我们应用前S1和前S2合成肽建立的一种酶免疫技术检测104例急慢性HBV感染者的血清。前S1和前S2抗体的检出率在18例急性乙型肝炎中为100.0％和66.7％,在50例无症状HBV携带者中为30.0％和24.0％,在36例慢性活动型肝炎中为47.2％和19.4％,在无症状HBV携带者和慢性活动型肝炎中前S抗体检出率在无病毒复制时高于病毒活跃复制期。结果提示:在急性HBV感染者中前S抗体较抗HBs更早出现,可能是疾病早期预后状况和感染恢复的标志,慢性HBV感染者中前S抗体不仅存在,且可反映病毒清除。

Anti-pre-S responses in implication of hepatitis B viral clearance and in predication of clinical course

Enzyme immunoassays were established to study the clinical significance of anti-pre-Sl and antipre-S2 .using synthetic oligopepetide analogues of pre-S1 and pre-S2 proteins as reagent antigens, and 104 cases of different categories of HBV infection were tested. The prevalence of anti-pre-S1 and anti-pre-S2 were 100% and 66. 7% in eighteen cases of acute hepatitis B; 30. 0% and 24. 0% in fifty-five cases of asymptomatic HBV carriers; and 47. 2% and 19. 4% in thirty six cases of chronic active hepatitis, respectively. The detection rate of anti-pre-Sl and anti-pre-S2 during nonvirus leplication stage was higher than that of active-virus replication stage in AsC and CAH. The results suggested that in acute hepatitis B virus infection anti-pre-S appeared even earlier than anti-HBs and might be used as markers for early recovery of the infection. Anti-pre-S might reflect viral clearance in chronic infections.
