紫杉醇加顺铂联合放疗同步治疗局部晚期非小细胞肺癌的临床研究

目的评价紫杉醇(PTX)加顺铂(DDP)联合放疗同步治疗局部晚期非小细胞肺癌(NSCLC)的临床疗效和不良反应。方法60例局部晚期NSCLC患者完全随机分为联合组(31例)和化疗组(29例)。化疗组单纯采用PC方案,其中PTX135mg/m2,d1;DDP75mg/m2,d1。联合组在PC方案化疗基础上,同时进行常规放疗。结果60例患者全部完成治疗计划,联合组总有效率(87.1%)明显高于化疗(55.2%),P<0.05。联合组的1、2年生存率分别为78.5%和50.6%,化疗组分别为51.2%和30.5%,差异有统计学意义(P<0.05)。联合组不良反应稍高于化疗组,但差异无统计学意义(P>0.05)。结论紫杉醇加顺铂联合放疗同步治疗局部晚期NSCLC有较好的近期疗效,可提高生存率,毒性反应可耐受。

Efficacy of concurrent radio-chemotherapy and chemotherapy alone in the treatment of locally advanced non-small-cell lung cancer

Objective To evaluate the effect and acute toxicity of concurrent radio-chemotherapy, by PTX and DDP, plus concurrent radiotherapy in comparison with chemotherapy alone in the treatment of inoperablelocally advanced non-small-cell lung cancer (LANSCLC) . Methods Sixty patients with inoperable LANSCLC were randomly divided by envelope method into two groups:concurrent radio-chemotherapy group ( n =31) and conventional chemotherapy group ( n = 29) . The patients in conventional chemotherapy group were treated by PC regimen(PTX DDP) : PTX 135 mg/m2, d1 and DDP 75 mg/m2 d1. In the radio-chemotherapy group by PC regimen plus conventional radiotherapy. Results All patients completed the treatment schedule. The overall response rate (CR PR) in the radio-chemotherapy group was significantly higher than that in the conventional chemotherapy group:87.1 % vs 55.2 %( P <0.05) with 1- and 2-year survival rates of 78.5 % vs 50.6% (P <0.05) and 51.2 % vs30.5%( P < 0.05) .without any significant difference in the acute toxicity between two groups (P >0.05) . Conclusion Compared with conventional chemotherapy (PTX and DDP) , concurrent radio-chemotherapy (PTX and DDP plus concurrent radiotherapy) is more effective and tolerable for the inoperable locally advanced non-small-cell lung cancer.
