超声定位支架引导肾组织活检在陕北地区的应用

目的提高陕北地区对弥漫性肾病诊治水平。方法在超声引导下对26例弥漫性肾病患者经皮快速自动肾穿刺活检。结果26例肾穿刺均成功取材,成功率为100%。其中18例肾穿刺取材两针成功,另6例一针穿刺成功,2例3针穿刺成功;穿刺出标本长度为6～22mm,并行光镜、免疫荧光,必要时加做电镜、乙肝标志物等。结论超声引导下经皮自动肾穿刺活检简便、易行,取材满意,可为临床病理学诊断提供理想的活检标本。经病理组织活检确诊微小病变型肾病3例,轻度系膜增生性肾小球肾炎7例,膜性肾病5例,IgA肾病2例,膜增生性肾小球肾炎1例,局灶增生性肾小球肾炎2例,ANCA相关性小血管炎1例,乙肝病毒相关性肾炎3例,过敏性紫癜性肾炎1例,糖尿病肾小球硬化1例。

Application of renal biopsy by ultrasound positioning and stent guided in northern Shaanxi Province

Objective To improve diagnosis and treatment of diffuse kidney diseases in northern Shaanxi province.Methods 26 patients with renal diffuse diseses were penetrated by rapid automated percutaneous renal biopsy by ultrasound-guided.Results renal bio-sy of 26 cases were successfully,success rate was 100%.In all cases,18 cases of renal biopsy successful puncture twice;other 6 cases only once;2 cases there times.a specimen length about 6~22mm by puncture,examination with optical microscopy,immunofluorescence.if necessary,examined by electron microscopy,hepatitis B markers,etc.Conclusions percutaneous renal biopsy automatically by Ultra-sound guided is simply,easily and satisfy,contributed to ideal biopsy materials for clinical and pathological diagnosis.3 minimal change nephrotic syndrome cases were patholo-gical diagnosised by biopsy,7 cases were mild mesangial proliferative glomerulo-nephritis,5cases were membranous nephropathy,2 cases were IgA nephropathy,1 cases were membranoproliferative glomerulonephritis,2 cases were focal prolifera-tive glomerulonephritis,1 case was ANCA associated minimal vessel vasculitis,3 cases were glomerulonephritis associated hepatitis B virus infection,1 case was allergic purpura nephritis,1 case was diabetic glomerular sclerosis.
