急性颅脑损伤患者脑脊液脑型肌酸激酶同工酶测定的诊断作用

采用层析荧光法对62例急性颅脑损伤患者CSF-CK-BB活性作了测定.结果表明,急性颅脑损伤患者伤后早期CSF-CK-BB活性明显升高,并与脑损伤严重程度和患者预后相关,而与颅内压增高无关.CSF-CK-BB活性与伤情估计GCS评分成反比例关系.单纯颅脑伤与颅脑复合伤CSF-CK-BB活性无差异.我们认为,急性颅脑损伤患者伤后早期CSF-CK-BB活性测定可以作为一项新的评定脑损伤严重程度和患者预后的诊断参考指标.

Diagnostic suggestion of measuring brain creatine kinase activity in CBF of patients with acute head injury

The brain type creatine kinase isoenzyme (CK -BB)activity in the cerebrospinal fluid (CSF)of 62 patients with acute head injury was measured by means of chromatographic separating and fluoro-metric quantifying.The results showed that the CSF -CK-BB activity of the patients with acute head injury was much higher than that of the patients with control groun and there was a good corre- lation between CSF-CK-BB activity and the severe degree of the brain damage as well as the outcome of the patients following trauma,but there was no corrilation between CSF-CK-BB activity and intracranial pressure there was an inverse correlation between CSF-CK-BB activity and Glasgow ComaScale (GCS)of neurological disfunction,There was also no signifcant correlation between simple head injury and multiple trauma.Consequently,it is considered that the test of the CSF-CK-BB activity can be used as an new diagnostic index for assessing the severity of brain damage and the prognosis of the patients after acute head injury .
