胰腺癌多层螺旋CT灌注成像的临床研究

摘    要:目的应用多层螺旋CT(MSCT)灌注扫描成像技术评价胰腺癌的血流动力学表现并探讨其临床应用价值。方法采用GE Lightspeed 16层螺旋CT ADW4.1工作站,使用Perfusion-3软件包分析。测量血流量(BF)、血容量(BV)、平均通过时间(MTT)和毛细血管表面通透性(PS)。结果胰腺癌组的BF、BV明显小于正常组,PS、MTT大于正常组。胰腺癌组与正常组之间的BF、BV、MTT和PS均有显著的统计学意义。胰腺癌的血流灌注参数中BF、BV、PS值随着Hemrick CT标准分期的增高而明显增加,呈明显的正相关。MTT值随着Hemrick CT标准分期的增高而明显下降,呈明显的负相关。结论MSCT灌注成像对胰腺癌血流变化的分析有一定临床价值。

Abstract:Objective To assess the change of the blood flow for pancreatic cancer and discuss its clinical value using MSCT perfusion technology. Methods The CT perfusion imaging was obtained by using GE Light speed 16 slice spiral CT,and analyzed on ADW 4.1 Workstation with perfusion-3 software package. Hemedynamic parameters including blood flow(BF),blood volume(BV),mean transmit time(MTT) and capillary vessel permeability surface(PS) were calculated. Results BF and BV of pancreatic cancer were obviously lower than in normal pancreas ,but MTT and PS were higher than in normal pancreas. There was statistically significant difference in BF,BV,MTT and PS between the groups of normal pancreas and pancreatic cancer. BF,BV,and PS of the blood perfusion parameter of the pancreatic cancer obviously increased along with the increment of Hemrick's CT period with obviously positive correlation. But MTT descended obviously along with the increment of Hemrick's CT period with obviously negative correlation. Conclusion MSCT perfusion imaging is of clinical significance to analyzing the change of the blood flow for pancreatic cancer.
