益气化瘀盆炎汤加减治疗盆腔炎气虚血瘀证疗效及对患者血液微循环的影响

目的:探讨益气化瘀盆炎汤加减治疗盆腔炎气虚血瘀证临床疗效及对患者血液微循环的影响。方法:将96例盆腔炎气虚血瘀证患者,随机分为对照组与观察组,各48例。对照组给予左氧氟沙星联合桂枝茯苓胶囊治疗,观察组在对照组治疗基础上给予益气化瘀盆炎汤加减治疗,连续用药2个疗程。比较两组患者临床疗效、治疗前后的中医症候积分、炎性包块直径、盆腔积液深度、血液微循环指标、炎症因子水平、补体C₃和C₄水平,及用药期间的不良反应发生情况。结果:观察组临床治愈率较对照组更高,组间比较差异均有统计学意义(P<0.05)。观察组治疗后的中医症候积分、炎性包块直径、盆腔积液深度均较对照组下降更显著,观察组治疗后的血清ICAM-1、D-二聚体、血浆黏度、红细胞沉降率、血小板聚集率水平均较对照组下降显著,IL-2、IL-10、MMP-2水平均较对照组明显提高,组间比较差异均有统计学意义(P<0.05)。观察组治疗后的补体C₃和C₄水平均较对照组高,组间比较差异具有统计学意义(P<0.05)。两组用药过程中均未见肝肾功能损伤等严重不良反应发生。结论:益气化瘀盆炎汤加减治疗盆腔炎气虚血瘀证疗效较好,其机制可能与改善患者血液微循环、免疫功能及控制炎症因子水平有关。

Effect of modified Yiqi Huayu Penyan decoction on treatment of pelvic inflammation of Qixu Xueyu syndrome and its influence on blood microcirculation

Objective:To explore the clinical effect of Yiqi Huayu Penyan decoction in treatment of pelvic inflammatory disease with Qixu Xueyu syndrome and its influence on blood microcirculation of patients.Methods:96 patients with pelvic inflammatory disease with Qixu Xueyu syndrome were randomly divided into control group and observation group,with 48 cases in each.The control group was treated with levofloxacin combined with Guizhi Fuling capsules,and the observation group was treated with Yiqi Huayu Penyan decoction on the basis of the treatment of the control group.The treatment was continued for 2 courses.Statistics of clinical efficacy,TCM syndrome score before and after treatment,sex mass diameter,pelvic effusion depth,blood microcirculation indicators,inflammatory factor levels,complement C₃ and C₄,and count the occurrence of adverse drug reactions during patients' medication.Results:The clinical cure rate of the observation group was significantly higher than that of the control group,difference between groups statistically significant(P<0.05).The observation group's TCM syndrome score,inflammatory mass diameter,and pelvic effusion depth after treatment were better than those of the control group.Serum ICAM-1,D-dimer,plasma viscosity,erythrocyte sedimentation rate,and platelet aggregation rate of the observation group after treatment were lower than those of the control group,and serum IL-2,IL-10,MMP- 2.The level was higher than that of the control group,difference between groups statistically significant(P<0.05).After treatment,C₃ and C₄ levels of the observation group were significantly higher than those of the control group,difference between groups statistically significant(P<0.05).There were no serious complications such as liver and kidney function damage in the two groups.Conclusion:Yiqi Huayu Penyan decoction has satisfactory effect in treatment of pelvic inflammatory disease with Qixu Xueyu syndrome,and can significantly improve the clinical efficacy.The mechanism of action may be related to improving the blood microcirculation and immune function,control the levels of inflammatory factor.
