加权综合指数法在临床科室考核中的应用与分析

目的应用加权综合指数法考核临床科室，为医院管理提供依据。方法选取2014年-2018年16个临床科室的出院人数、病床使用率、病床周转次数、出院患者平均住院日、住院患者人均医药费用、住院药占比、总诊疗人次数、门诊次均费用、门诊药占比9个考核指标，将反向指标正向化、部分指标均值化。采用Z极值法标准化指标、因子分析法计算指标权重。结果病床周转次数、总诊疗人次数、病床使用率、住院药占比为重要指标，权重分别为0.966、0.931、0.926和0.904。加权指数值较Z值指数值结果可靠。外科综合指数值较内科高；胸外科与重症肝病科、感染科、血友病感染科自建科以来，综合指数均为负值。结论加权综合指数法适用于临床科室考核。为加快医院转型发展，应给予新建科室、优势科室病床数、科研、教学支持。

Application and Analysis of Weighted Comprehensive Index Method in Clinical Department Evaluation

ObjectiveTo use weighted comprehensive index method to evaluate clinical departments and provide decision for hospital management. MethodsThe 9 evaluation indexes from 16 clinical departments in 2014-2018 were selected including the discharged number, hospital bed utilization rate, bed turnover times, average length of stay, medical expenses per inpatient, proportion of inpatient drugs, total number of patients, average cost per outpatient, and proportion of outpatient drugs. The reverse index was positively transformed and some indexes were averaged. The Z extremum method was used to standardize the index and factor analysis method was used to calculate the index weight.ResultsThe hospital bed turnover times, the total number of patients, the utilization rate of hospital bed and the proportion of inpatients drugs were the important indexes, with the weights of 0.966, 0.931, 0.926 and 0.904, respectively. The weighted index value was more reliable than the Z-value index value. The comprehensive index of surgery department was higher than that of internal medicine department. The comprehensive index of the department of thoracic surgery and the department of severe liver disease, the department of infectious diseases, and the department of hemophilia infection were all negative since the department was established. Conclusion The weighted comprehensive index method was suitable for evaluation of clinical department. In order to accelerate the transformation and development of the hospital, it should give beds, scientific research and teaching support to the new departments and the superior departments.
