经脐单切口腹腔镜在子宫肌瘤剔除中的应用

目的探讨经脐单切口腹腔镜子宫肌瘤剔除的临床应用价值。方法对行腹腔镜子宫肌瘤剔除的46例患者进行回顾性分析，其中行经脐单切口腹腔镜子宫肌瘤剔除的病例19例（经脐单切口腹腔镜组），行多孔腹腔镜子宫肌瘤剔除的病例27例（多孔腹腔镜组），比较两组患者各项手术指标间的差异。结果两组手术均无副损伤，同时两组患者的手术时间、术中出血、中转开腹例数、术后感染例数、术后住院时间以及住院费用差异均无统计学意义（P>0.05）；而经脐单切口腹腔镜组术后排气时间早于多孔腹腔镜组（P<0.05），同时术后疼痛评分低于多孔腹腔镜组（P<0.05）。结论经脐单切口腹腔镜子宫肌瘤剔除术的临床疗效与多孔腹腔镜子宫肌瘤剔除术相当，且切口更微创，恢复更快捷，值得在临床推广应用。

The application of the transumbilical single-incision laparoscopic surgery for myomectomy

Objective To compare the differences in hysteromyomectomy between the transumbilical single-incision laparoscopic surgery(TU-SILS) and multi-port laparoscopic surgery(MPLS), thus to explore the clinical application value of TU-SILS. Methods Forty-six patients with laparoscopic myomectomy hospitalized were retrospectively analyzed, including nineteen patients with TU-SILS and twenty-seven patients with MPLS. The differences of the two groups were compared with various operation indicators. Results There was no operative collateral injury in either groups. And there was no significant difference observed in operation time, intraoperative bleeding, transit number of open operation, postoperative infection,length of postoperative hospital stay or hospitalization expenses between TU-SILS group and MPLP group (P>0.05). At the same time, compared to MPLS group, postoperative exhaust time of TU-SILS group was earlier (P<0.05), and postoperative pain score of TU-SILS group was lower (P<0.05). Conclusion The clinical curative effect of TU-SILS in hysteromyomectomy is equal to MPLS. However, TU-SILP provides more minimally invasive incision and faster recovery.
