活血定喘汤对支气管哮喘患儿气道炎症及炎性因子水平的影响

目的:探讨活血定喘汤对支气管哮喘患儿气道炎症及炎性因子水平的影响。方法:将诊断为支气管哮喘的98例患儿,随机分为研究组和对照组各49例,对照组口服孟鲁司特钠片治疗,研究组在对照组基础上联合加用活血定喘汤治疗,比较两组患儿临床治疗总有效率和不良反应发生率,肺功能改善情况,同时检测并比较两组患者治疗前后的血清炎症因子白细胞介素13(IL-13)、白细胞介素-4(IL-4)、γ-干扰素(IFN-γ)]、免疫细胞水平嗜酸性粒细胞直接计数(EOS)、嗜酸性阳离子蛋白(ECP)、免疫球蛋白 E(IgE)、CD₄⁺、CD₄⁺/ CD₈⁺]以及血清1-磷酸鞘氨醇(S1P)、磷脂酰肌醇-3-激酶(PI3K)表达水平。结果:治疗3个月后同组患儿FEV₁、FEV₁/FVC以及C-ACT评分均显著高于治疗前,而研究组的改善及变化幅度明显高于对照组(P<0.05); 两组患儿治疗的有效率差异具有统计学意义(P<0.05),研究组为89.80%,明显高于对照组的65.31%; 同组患者比较,治疗后的血清炎性因子(IL-4、IL-13)、免疫细胞水平(ECP、EOS、IgE、CD₈⁺)、S1P水平以及PI3K表达水平均显著低于治疗前,而CD₄⁺、INF-γ和CD₄⁺/ CD₈⁺则高于治疗前,且与对照组相比,研究组变化更明显(P<0.05); 两组患儿不良反应发生比较,差异无统计学意义(P>0.05)。结论:活血定喘汤治疗儿童支气管哮喘临床疗效显著,可改善患儿体内血清炎症因子、PI3K以及 S1P水平,增强患儿肺功能和免疫功能。

Effects of Huoxue Dingchuan decoction on airway inflammation and inflammatory factors in children with bronchial asthma

Objective:To explore the effect of Huoxue Dingchuan decoction on airway inflammation and inflammatory factors in children with bronchial asthma.Methods:The subjects were 98 cases of children with bronchial asthma.All children were randomly divided into study group(49 cases)and control group(49 cases).The control group was treated with montelukast sodium tablets,and the research group was treated with Huoxue Dingchuan decoction in combination with the control group.The total effective rate of clinical treatment,incidence of adverse reactions,and improvement in lung function were compared between the two groups of children.At the same time,the serum inflammatory factors Interleukin 13(IL-13),Interleukin-4(IL-4),γ-interferon(Interferon-γ,IFN-γ)],Immune cell level Direct eosinophil count(EOS),Eosinophil cationic protein(ECP),Immunoglobulin E(IgE),CD₄⁺,CD₄⁺/ CD₈⁺] and serum sphingosine 1-phosphate(S1P),phosphatidylinositol-3-kinase(PI3K)expression levels detected and compared.Results:After three months of treatment,the scores of FEV₁,FEV₁ / FVC and c-act in the same group were significantly higher than those before treatment,while the improvement and change range of the study group were significantly higher than those of the control group(P<0.05); the effective rate of treatment between the two groups was statistically significant(P<0.05),89.80% in the study group,significantly higher than 65.31% in the control group.Compared with the control group,the levels of serum inflammatory factors(IL-4,IL-13),immune cell levels(ECP,EOS,IgE,CD₈⁺),S1P and PI3K expression levels after treatment were significantly lower than those before treatment,while the levels of CD₄⁺,INF-γ and CD₄⁺/ CD₈⁺ were higher than those before treatment.Compared with the control group,the study group had more changes(P<0.05).The difference of adverse reactions between the two groups did not reach the statistical standard(P>0.05).Conclusion:Huoxue Dingchuan decoction has a significant clinical effect on children with bronchial asthma,which can significantly improve the serum inflammatory factors,PI3K and S1P levels in children,and enhance lung function and immune function.
