2016-2019苏州市副溶血性弧菌的毒力基因和耐药性及分子分型研究

摘    要:目的 了解2016-2019年苏州市副溶血性弧菌( Vibrio parahaemolyticus ,VP)分离株毒力基因携带情况,耐药性及分子分型。 方法 利用荧光定量PCR 技术对154株分离自苏州各区、县级市的VP的3种毒力基因( tlh 、 tdh 、 trh )进行检测。采用微量肉汤稀释法测定其对14 种抗生素的耐药性。采用脉冲场凝胶电泳(Pulsed-Field Gel Electrophoresis, PFGE)进行分子分型。采用卡方检验对统计结果进行分析( α =0.05)。 结果 154株VP分离株 tlh 基因均为阳性; trh 基因阳性4株,阳性率为2.60%,均为病患分离; tdh 基因阳性107 株,阳性率为69.48%,其中病患分离株阳性106株,食品及环节(用具)分离株阳性1株。154株VP仅对氨苄西林耐药,总体耐药率为80.52%(124/154),其中病患分离株耐药率为88.98%(105/118)高于食品及环节分离株耐药率52.78%(19/36),差异具有统计学意义( P <0.001)。154株VP 经PFGE分子分型显示带型相似性为45%~100%,按相似度85%为判读标准可分为73个带型簇(各带型簇:1≤n≤10)。 结论 苏州市VP的 trh 基因的携带率较低,病患分离株的 tdh 基因的携带率较高,远高于食品及环节分离株。VP分离株对多数抗生素不耐药,仅对氨苄西林存在较高的耐药率。PFGE带型较为分散,没有明显的优势带型簇。

Abstract:To investigate virulence gene carrying status, drug resistance and molecular typing of Vibrio parahaemolyticus(VP) strains in Suzhou from 2016 to 2019. Methods Three virulence genes(tlh, tdh, trh) of 154 VP strains isolated from various districts and county-level cities in Suzhou were detected by real-time PCR. The resistance to 14 kinds of antibiotics was determined by the broth microdilution method. Pulsed-Field Gel Electrophoresis(PFGE)was used for molecular typing. Chi-square test was used to analyze the statistical results(α=0.05). Results 154 VP isolates were all found carrying tlh gene. 4 trh gene positive strains were all isolated from patients, with a positive rate of 2.60%. There were 107tdh gene positive strains with a positive rate of 69.48%, of which 106 strains were isolated from patients and 1 from food and link(appliance). 154 strains of VP were only resistant to ampicillin, and the drug resistance rate of the isolates from patients was 88.98%(105/118), which was higher than that of food and link isolates 52.78%(19/36), and the difference was statistically significant(P<0.001). 154 strains of VP showed a band similarity between 45% and 100% by PFGE molecular typing. According to the similarity criterion of 85%, they could be divided into 73 band clusters(each band cluster: 1≤n≤10). Conclusion The carrying rate of trh gene of VP in Suzhou City is low, and the carrying rate oftdh gene in patient isolates is much higher than that in food and link isolates. VP isolates are not resistant to most antibiotics, but they have a high resistance rate only to ampicillin. The PFGE bands are dispersed, and there are no obvious dominant band clusters.
