老年男性骨质疏松患者血清IGF-1、IGFBP-3水平与骨代谢及颈动脉内膜中层厚度的关系

摘    要:目的探讨老年男性骨质疏松患者血清胰岛素样生长因子-1(IGF-1)、胰岛素样生长因子结合蛋白-3(IGFBP-3)水平与骨代谢及颈动脉内膜中层厚度(CIMT)的关系。方法选取106例老年男性骨质疏松患者作为研究对象,选取同期体检健康者110例作为对照组,观察两组受试者血清IGF-1、IGFBP-3水平、骨代谢指标及CIMT变化,并分析血清IGF-1、IGFBP-3水平与骨代谢指标、CIMT相关性。结果观察组患者血清IGF-1、IGFBP-3水平均明显低于对照组(P<0.05);观察组患者骨密度(BMD)、骨钙素(BGP)、I型前胶原氨基端原肽(PINP)水平低于对照组(P<0.05),抗酒石酸酸性磷酸酶(TRACP-5b)水平明显高于对照组(P<0.05);观察组患者CIMT增厚程度明显高于对照组(P<0.05)。相关性分析显示,骨质疏松患者血清IGF-1、IGFBP-3与BMD、BGP、PINP呈正相关(P<0.05),与TRACP-5b、CIMT呈负相关(P<0.05)。结论老年男性骨质疏松患者血清IGF-1、IGFBP-3水平降低,二者与BMD、BGP、PINP呈正相关,与TRACP-5b、CIMT呈负相关,IGF-1、IGFBP-3可作为老年男性骨质疏松诊断的参考指标。

Abstract:ObjectiveTo investigate the correlations between serum insulin-like growth factor-1 (IGF-1) and insulin-like growth factor binding protein-3 (IGFBP-3) levels and bone metabolism and carotid intima-media thickness (CIMT) in elderly male patients with osteoporosis. Methods Totally 106 elderly male osteoporosis patients admitted to our hospital from March 2017 to March 2019 were selected, and 110 healthy persons in the same period were selected as control group. The levels of serum IGF-1, IGFBP-3, bone metabolism indexes and CIMT were observed, and the correlations between levels of serum IGF-1, IGFBP-3 and bone metabolism, CIMT were analyzed. ResultsThe levels of serum IGF-1 and IGFBP-3 in the study group were significantly lower than those in the control group (P<0.05). Bone mineral density (BMD), osteocalcin (BGP), and procollagen Type I amino terminal peptide (PINP) levels in the study group were significantly lower than those in the control group (P<0.05); and tartrate-resistant acid phosphatase (TRACP-5b) level was significantly higher than that in the control group (P<0.05). The thickness of CIMT in the study group was significantly higher than that in the control group (P<0.05). The correlation analysis showed that serum IGF-1 and IGFBP-3 were positively correlated with BMD, BGP and PINP in patients with osteoporosis (P<0.05), and negatively correlated with TRACP-5b and CIMT (P<0.05). ConclusionThe levels of serum IGF-1 and IGFBP-3 in elderly male patients with osteoporosis are decreased, and they are positively correlated with BMD, BGP and PINP; and negatively correlated with TRACP-5b and CIMT. IGF-1 and IGFBP-3 can be used as reference indexes for the diagnosis of osteoporosis in elderly men.
