腮腺巨大多形性腺瘤一例

多形性腺瘤是最常见的唾液腺肿瘤,然而超过1 kg的腮腺巨大多形性腺瘤则极为罕见。该文报道了1例71岁男性腮腺巨大多形性腺瘤患者的诊治经过,该例患者因左侧耳下区渐大包块35年入院,入院完善相关检查后行包块探查切除术。术中见一大小约160 mm×130mm×140 mm的肿物,术后肿物称重1.55 kg,病理回示符合腮腺巨大多形性腺瘤。术后随访6月,患者左侧颌面部外观明显改善,无神经损伤症状。该例的诊治过程提示,腮腺多形性腺瘤瘤体巨大,手术过程中应注意避免损伤邻近神经血管,临床医师应加强对本病的认识,提升诊治水平。

Giant parotid pleomorphic adenoma: one case report

Pleomorphic adenoma is the most common type of salivary gland tumors. However, giant parotid pleomorphic adenoma with a weight over 1 kg is extremely rare. In this article, the diagnosis and treatment of one 71-year-old male patient with giant parotid pleomorphic adenoma were reported. The patient was admitted to hospital due to an enlarged mass in the left subauricular area for 35 years. After complete relevant examinations, the patient underwent exploratory resection of the mass. Intraoperatively, a mass of 160 mm×130 mm×140 mm in size with a weight of 1.55 kg was observed. The postoperative pathological results were consistent with the diagnosis of giant parotid pleomorphic adenoma. During 6-month follow-up, the left maxillofacial appearance of the patient was significantly improved, and no symptoms of nerve injury were reported. The diagnosis and treatment of this case prompt that extensive attention should be paid to avoid surrounding neurovascular injury during the operation. Clinicians should strengthen the understanding of this disease and improve the level of diagnosis and treatment.
