摘    要:目的 了解江阴市流感样病例(influenza like illness, ILI)病原谱的构成及流行特征,为制定防治策略提供科学依据。 方法 采集2016-2017年江阴市流感监测哨点医院上报的流感样病例咽拭子标本及病例信息,运用实时荧光定量PCR技术对标本中的流感病毒、副流感病毒、常见冠状病毒、呼吸道合胞病毒、鼻病毒、偏肺病毒、呼吸道腺病毒、肺炎衣原体及支原体进行检测,应用Epi Info对采集的数据进行统计分析。 结果 2016-2017年全市累积报告ILI 64305例,占就诊病人总数的百分比(ILI%)为9.20%,各年份ILI%差异有统计学意义(P<0.001),ILI%具有明显的季节性,且0~4岁组ILI报告例数最多。2016-2017监测年度内,2家监测哨点医院共采集咽拭子标本1059份,检出病原阳性率为38.62%,流感病毒为本地区的优势病原(阳性率23.98%),其次为呼吸道腺病毒(阳性率5.38%)和副流感病毒(阳性率2.46%)。5~14岁组病原学阳性率最高(53.57%),4月份为呼吸道病原感染高峰(67.06%)。结论 江阴市ILI呼吸道病原种类多样,流感病毒为主要病原,高发季节在冬春季,15岁以下人群为高危人群。因此,在重视流感防控的同时,要加强其他呼吸道病原的防治。

Abstract:To study the composition and epidemic characteristics of ILI in Jiangyin during 2016-2017 and to provide scientific references for developing the strategies of prevention and control. Methods Pharyngeal swab samples and case information of ILI were collected by influenza surveillance sentinel hospital. Real-time PCR was used to detect Flu, HPIV, HCoV, ADV, RSV, hMPV, HRV, CP and MP. Epi Info was used for the statistical analysis. Results 64305 ILI cases were reported in the city from 2016 to 2017, accounting for 9.20% of the total patients(ILI%). During 2016-2017, the 1059 samples of pharyngeal swabs were collected from 2 sentinel hospitals, and the pathogen positive rate was 38.62%, and Flu was the dominant pathogen in this region(the positive rate was 23.98%), followed by ADV(5.38%) and HPIV(2.46%). The pathogen positive rate(53.57%) was the highest in 5~14 years old group, and the peak of respiratory tract pathogen infection was in April(67.06%). Conclusion There are various respiratory pathogens in ILI cases in Jiangyin city. Flu is the main pathogen and the high incidence season is winter and spring. Therefore, while paying attention to the prevention and control of influenza, we should strengthen the prevention and control of other respiratory pathogens.
